Family Living

The Family Living Service is a community living service offered under the DD Waiver sponsored by the New Mexico Department of Health Developmental Disabilities Support Division.  The chief intent is to provide individuals with developmental disabilities support services in a residential setting in order to increase, maintain or promote the individual’s capacity for independent functioning, self-determination, self-advocacy, productivity, and community integration.

The Family Living Service entails the placement of an individual in the home of a Surrogate or Natural Family Provider who accepts and cares for this individual as if he/she was a family member.

Aside from cultivating a long-term relationship with the individual, responsibilities include:

  • Home safety

  • Meal planning/preparation

  • Money management

  • Personal  assistance

  • Dispensing medication

  • Community integration

  • Transportation

Active Solutions Incorporated actively recruits, approves, trains, and supports these direct support providers.  ASI establishes the best possible match between the individual and the direct support provider by offering to do and meet and greet.  Additionally, ASI recruits and approves respite providers. Family Living providers are encouraged to use all allotted substitute care.

It’s a team effort

The care and welfare of a person with developmental disabilities requires the collaboration and cooperation of all parties involved.

Active Solutions Incorporated is committed to working with case managers, guardians, families, and all Interdisciplinary Team Members to properly advocate for and support each individual we serve.
